Billing and service charges...

Office co-payments are expected to be paid at the time of your visit.  Please present all up-to-date insurance cards and a government-issued form of photo identification at the time of your visit.  Without an updated/valid insurance card from one of the plans with which we participate, payment in full for your visit will be due a the time that services are rendered.

All outstanding balances that are not covered by your insurance plan are expected in full within 30 days of receiving your billing statement.  Because we utilize ADO Billing Services, a third-party company to assist us with our billing, sometimes answers and resolutions to questions may not be available on the same day as your inquiry.  For more information, please call (718) 849-2453.  For inquiries regarding laboratory charges, please refer to the phone number(s) on your bill as they are handled separately by either Shiel Medical Laboratory or Quest Diagnostics.

The following administrative service fees may apply and are expected at the time of your visit:

 •  $25.00 for the completion of any form that is not associated with an office visit.

 •  $0.75 per page for copying or printing of records.  (There is no charge associated with faxing records to other medical providers)

In addition to cash and personal checks, we accept credit card payments via VISA, MasterCard, American Express, and Discover.  We also accept ApplePay and other NFC technologies in the office.

We will NOT be participating in the new exchange plans.

We do NOT participate in any managed Medicare plans.

We do NOT participate in Medicaid.

Manhattan Internal Medicine, PLLC's participation with insurance plans is subject to change without notice.

For plans with which we do not participate, a HCFA 1500 form is commonly required to receive reimbursement from your insurer.

  • the type of patient (new or established)
  • type of visit (screening physical exam, follow-up, and/or immediate problem)
  • other services provided (phlebotomy, immunizations, EKG, ear irrigation, foreign body removal, etc.).  
Although we can approximate the cost of the services, ultimately it depends upon the individual patient's history and examination at the time of his or her visit to see that the best possible care is provided.
